Purpose and Significance of Study: This is the report of a phase I/II clinical trial, containing very promising findings. Previous preclinical studies by the authors had showed that the pan histone deacetylase inhibitor vorinostat in combination with lapatinib significantly reduced the self-renewal capacity of cancer stem cells (CSCs) and prevented metastasis. The results of this trial show that this combination is safe and effective in HER-2 positive breast cancer. However, the study is limited by its small sample size (12). Nevertheless, the results indicate the need for novel clinical trial endpoint and correlative studies in order to assess new investigational agents targeting the CSC population.

 

Fit with Scope of Journal: The manuscript is of very high interest for the journal Clinical Oncology and Research.

 

The results are very clearly described, supported by the data. The Discussion aptly covers the implications of the findings and lays down the direction of future research in this area.

 

o   The manuscript contains a few minor errors (already corrected in the galley proof, except the major highlighted one, which requires the author’s attention) listed below –

 

·       The figures, including supplemental figures are missing.

·       In the 3rd sentence in the Results in the Abstract, “the” before “phase I” must be replaced with “in”, and in the next sentence, “positve” must be replaced with “positive”.

·       In the 1st sentence of the Statement of Significance, “,” after “CSCs” must be replaced with “’”.

·       In the 2nd sentence of Phase II cohort in Patient selection in the Materials and Methods, “providing” must be replaced with “provided”.
