§ Significance of the Study: This study highlights the role of tetrandrine (TTD) as a drug resistance reversal agent in acute myeloid leukemia (AML) and compares the effects of TTD combined with adriamycin (ADM) on the reversal of multidrug resistance (MDR) in K562/ADM cells. This study shows that TTD can significantly antagonize P-gp-mediated ADM resistance by competitively binding P-gp and down-regulating P-gp expression; this effect will not exist when combined with chemotherapeutic drugs that are more likely to bind to P-gp, but this suggests that MDR reversal agents can be found in P-gp high-affinity substrates. The role of the competitive inhibition of TTD with other chemotherapeutic drugs requires further studies.
§ Fit with the Scope of the Journal: The manuscript perfectly fits with the scope of the journal Clinical Oncology and Research.
§ The results of the performed experiments are thoroughly analysed in the Discussion section. The Conclusion is consistent with the evidences presented in the article.
§ A few minor errors (already corrected in the galley proof) are listed below:
o Table 7 is not given but mentioned in the text section of the article. Figures 3, 7 and Table 5 are given but not mentioned in the text.
o The last sentence of the Abstract should be rephrased as “co-administration of TTD, as a drug resistance reversal agent with ADM may be a better administration in the clinic.
o Words like “tetrandrine”, “BIO-RAD”, “Reagent”, “Annealing”, “curve”, “further”, “significant”, “determining” are misspelled in places.
o In the 1st sentence of the 2nd paragraph of The sensitivity of cells to drugs was observed by CCK-8 method, the sentence “Calculating the growth inhibition rate of cells” should be rephrased as “Calculation of the growth inhibition rate of cells”.
o The 2nd sentence of the 2nd paragraph of The sensitivity of cells to drugs was observed by CCK-8 method under Results section should be rephrased as “We chose the concentration of 1.0 μmol/l for TTD as a combined dose to reverse cell resistance”.
o The 2nd sub-heading of Results should be rephrased as “Determination of ADM Concentration Using Fluorescence Spectrophotometer”.
o In the 1st sentence of ATPase activity experiment under Results, “TTD effects to increase” should be rephrased as “TTD increases”.
o The last sentence of the 5th paragraph of the Discussion, “addition of” should be inserted before “TTD and ADM”.
