A: Very good
Comments: The case description is adequate. The Discussion section covers the rare clinical complications associated with cerebral abscess caused by Actinomyces odontolyticus. It elaborates on the rationale behind the importance of surgical interventions with long-term adjunctive therapy in the treatment of such rare invasive infection. This study also utilizes the literature relevant to this topic. The Conclusion section summarizes from the report that a combined surgical approach with neurosurgery and otolaryngology along with antibiotic therapy is required for the treatment of invasive actinomycosis.

Comments:
· The 3rd sentence of the 2nd paragraph of the Diagnostic Tests should be rephrased as “There was no growth in blood cultures.”
· In the 3rd sentence of the 2nd paragraph of the Postoperative Course, the sentences are fragmented and do not make sense. Hence, the sentences should be separated and rephrased.
· Words like “otolaryngology’, ‘enhancement” are misspelled in the text section of the manuscript.
Further comments on the paper
Comments: This case report describes a case of a 60-year-old immunocompetent male presented with cognitive impairment and speech difficulties along with receptive aphasia. Radiographic findings identified the location of temporal lobe abscess associated with Actinomyces odontolyticus. This case demonstrates the successful use of surgical intervention such as neurosurgery, otolaryngology, mastoidectomy followed by antibiotic and anticonvulsant therapy for drainage of the abscess, repair of the middle fossa defect and any source of infection which result in improvement in speech and cognition. This study suggests that a multidisciplinary approach is required to treat such rare cases.
Q: Would you recommend this manuscript for further publication?
