The patient in this case report underwent open radical right nephrectomy with no adverse perioperative events. The case description is adequate. The Discussion explains the challenges in treating the patient in the present case clearly. It also offers useful suggestions with respect to the treatment regimen. It offers a thorough and detailed analysis of the findings of the present study taking all the relevant literature into account. The study is well supported with briefly explained figures. The case report finally concludes that a delay in recognition and diagnosis of von-Hippel Lindau (vHL) syndrome can lead to increased morbidity and mortality from serious complications such as advanced renal carcinoma.

Comments: This case report presents the case of a 58-year-old Filipino male with unremarkable family history who presented with progressive right homonymous hemianopia. The study holds significance as it is the first documented case of a supratentorial hemangioblastoma in a Filipino with von-Hippel Lindau (vHL) and one of the very few reported in the country. Diagnosis of this disease condition is challenging as it involves a high degree of suspicion and performance of several diagnostic tests which can be physically and financially taxing to the patient. This case highlights the importance of maintaining a high index of suspicion to prompt search for other associated disease features in any patient presenting with a typical vHL lesion.
